Sabina Prajapati, PHC-NP, BScN, MScN

Sabina Prajapati is a dedicated Family Nurse Practitioner committed to delivering compassionate, evidence-based, and patient-centered care. With advanced clinical training in primary health care and extensive experience across long-term care, primary care, acute medicine, and oncology settings, she brings a strong foundation of clinical expertise, leadership, and advocacy to her practice. Sabina is passionate about advancing quality care while empowering patients and families through education and collaborative decision-making.
Currently practicing at Glen Hill Strathaven, Sabina provides comprehensive primary and acute care within the long-term care setting. She conducts thorough health assessments, orders and interprets diagnostic tests, formulates evidence-based treatment plans, and prescribes medications within her scope of practice. Her approach emphasizes chronic disease management, preventive care, and holistic support for residents with complex comorbidities. She works collaboratively with interdisciplinary teams, mentors nursing staff and students, and advocates for equitable access to quality healthcare services.
Sabina served as a Nurse Practitioner at Rapelje Lodge in the Niagara Region, also within long-term care. In this role, she performed comprehensive geriatric assessments, developed management plans, and applied advanced nursing competencies in clinical practice, research, leadership, and consultation. She played a key leadership role in improving quality of care and successfully contributed to reducing hospital transfer rates by proactively managing urgent resident needs. She also provided both formal and informal education to staff, students, and families, fostering a culture of continuous improvement.
Sabina’s experience in primary health care includes her role at Winterberry Family Medicine, where she diagnosed and managed acute and chronic conditions, promoted disease prevention and health education, and collaborated closely with interdisciplinary team members and specialists to ensure comprehensive patient care.
Prior to becoming a Nurse Practitioner, Sabina practiced as a Registered Nurse and Night Charge Nurse at Trillium Health Partners – Credit Valley Hospital. There, she managed complex medical, surgical, and oncology patients, demonstrated strong clinical judgment, contributed to discharge planning, and actively participated in quality improvement initiatives. Concurrently, she worked at Extendicare Brampton in long-term and restorative care, providing leadership, supervising staff, and supporting residents with complex medical and palliative needs through a person-centered approach.
Sabina earned her Master of Nursing – Primary Health Care Nurse Practitioner and Bachelor of Science in Nursing from York University. She also holds a Bachelor of Nursing from Purbanchal University and a Proficiency Certificate in Nursing from Tribhuvan University. Her dedication to excellence was recognized through her nomination for the Joan Lesmond Award by Care for Nurses in 2021.
Committed to lifelong learning, Sabina has completed extensive professional development in wound care, palliative care, cardiology nursing, dementia care, and clinical leadership through organizations including Pallium Canada, Hospice Niagara, Humber College, and Trillium Health Partners.
She is an active member of the College of Nurses of Ontario and the Registered Nurses’ Association of Ontario, reflecting her commitment to professional standards, advocacy, and the advancement of nursing practice in Ontario.
